Establishing an Account
Please call our office and we will send you an Information Packet which will include all of our catalogs, order forms, and prepaid mailing labels.
If you would like to send an order right away, you can also download our order forms and prepaid labels right here on our website.
After filling out the appropriate order form (s), please use the enclosed prepaid mailing labels to ship us your first order.
Once your order has been received, you will receive an account number and be mailed customized prescription order forms. We will also call to welcome you aboard, and answer any questions you may have.
Your orders will be shipped back to you via UPS® (or USPS® if you prefer). We provide shipping on all custom orthotic orders to and from our lab at a flat rate of only $2.50. We also offer delivery in the Minneapolis/St. Paul area.
